A switch not properly lined resulted in train Q650-09 (with CSX 410 leading) entering the Banks siding and striking train A737-10 (CSX 5810). The complete report can be found at: http://www.fra.dot.gov/downloads/safety/Accident_Investigation/2005/hq200506.pdf
A737-10, with CSX 5810, had died on the hours of service. The crew had lined the south siding switch for their move but failed to restore the switch for the main. A heavy fog, a heavy train and a grade kept the resulting head-on wreck from being much worse as Q650-09 entered the improperly lined switch and struck the standing train.
